Who owns the health center?
Pueblo Community Health Center is an independent nonprofit organization managed by a community Board of Directors. This independence means that our community—not a corporate institution—makes the decisions regarding the services we offer.

Where are you located?
We have twelve locations in Pueblo and Huerfano Counties, Colorado, to serve you. Most sites are easily accessible by public transportation. Click here for detailed information.

How do I make an appointment?
If you are a current patient, you can call (719) 543-8711 to make an appointment. If you need to register to become a patient or to renew your card, click here for our Walk-In Registration procedure or call (719) 543-8711. If you have an emergency, we will make arrangements for a doctor to see you right away.

What are your policies regarding patient confidentiality?
We are committed to protecting your privacy and have strict policies to ensure patient confidentiality. These policies meet all federal and state health requirements.

Who do you serve?
We do not turn anyone away for inability to pay. Our services are available to anyone who wishes to come here. We primarily serve residents of Pueblo County, Colorado.

Do you accept private insurance?
Yes. We accept many different forms of private insurance, including Anthem Blue Cross Blue Shield, Secure Horizons and United Health Care. Since this information sometimes changes, if you do not see your insurance carrier on the list, please call (719) 543-8711 to confirm that we accept your coverage.

Do you accept Medicaid and Medicare?

What are your sources of funding?
Pueblo Community Health Center needs help from people like you to make sure we can continue serving people in need of health care. We receive support from many organizations—like Pueblo County United Way, the Pueblo City-County Partnership Program, Bureau of Primary Health Care, and others—as well as individuals in businesses. What do I do if I can’t pay for my care?
Your ability to pay should not be a barrier to receiving the health care you need. If you don’t think you can pay for your health care, please call our Patient Accounts staff at (719) 543-8711 to make an appointment to see what options are available. You might be eligible to set up a payment plan or we might be able to make arrangements for you to pay for services on a sliding scale.

With which hospitals are you affiliated?
We are able to meet your individual needs if you require hospitalization.  We work most closely with St. Mary-Corwin Medical Center and Parkview Medical Center.
