Migrant/Seasonal Farmworker Services

Pueblo Community Health Center is proud to offer special services and rates for Pueblo County’s agricultural community. During annual peak growing seasons, we extend our hours of operation and visit migrant/seasonal farm workers in the fields of Eastern Pueblo County. We encourage preventive and routine physical exams, as well as appropriate immunizations. These special residents are especially susceptible to work-related infections and injuries, so Pueblo Community Health Center provides the education and direct services needed to protect the agricultural workers.

Homeless Services

Pueblo Community Health Center is the only health care provider in Pueblo County with a clinic dedicated to serving the homeless population. PCHC defines "homeless" very simply: lacking permanent housing. The individuals and families we serve at our homeless clinic may be found staying in homeless shelters, sleeping on the street or in a vehicle, in a transitional housing program or moving from place to place.

An individual may also be considered homeless if they are unable to maintain their housing situation and are forced to stay with a series of friends and/or extended family members. Individuals who are released from a prison or a hospital may be considered homeless if they do not have a stable housing situation to which they can return.

Pueblo Community Health Center is proud to offer health care services to the homeless people of Pueblo. Our Health Care for the Homeless program is located at 1008 N. Grand Avenue, Pueblo, CO 81003. We are a proud member of the National Healthcare for the Homeless Council.

HIV/AIDS Services

The Ryan White Title III, Early Intervention Services is for people infected with or at high risk of acquiring HIV/AIDS. Services are available at a reduced cost for low-income or uninsured patients.

Pueblo Community Health Center provides outpatient early intervention and primary medical care services for people with HIV/AIDS in Pueblo, Alamosa, Baca, Bent, Chaffee, Conejos, Costilla, Crowley, Custer, Fremont, Huerfano, Kiowa, Las Animas, Otero, Prowers, Rio Grande, and Saguache Counties.

Program Services include:

• Walk-in, confidential testing
• HIV counseling, testing and referral services
• Medical evaluation and clinical care
• Pharmaceutical care
• Oral health care
• Drug therapy adherence counseling
• Nutritional counseling
• Outpatient mental health care
• Outpatient substance abuse counseling
• Referrals and coordination with specialty care
• Coordination with Social Services
